Daily notes for OT and COTA therapists
Type the session in clinical shorthand, abbreviations, full sentences, or any mix. Brevara writes the note for each session, with appropriate CPT billing codes and CMS 8-minute rule units. No format to learn.
What you type
Session details that contain no patient-identifying information — clinical shorthand, abbreviations, full sentences, or any mix — plus your treatment times. There is no format to learn.
What comes back
A complete, billing-ready therapy note with appropriate CPT billing codes and CMS 8-minute rule unit calculations: the note for that session — daily, treatment, or visit note, whatever your setting calls it. Copy it and paste it into any EHR. Need to fix a detail or add a time? Send a follow-up and Brevara updates the note without starting over.
Example: one COTA session

97535/Self-care and ADL training x 38 mins
Session conducted in the patient's home. The COTA initiated ADL training focused on showering with attention to infection control precautions, fall risk reduction, and maximizing independence with self-care tasks. Instructed pt in energy conservation techniques during showering to reduce exertion demands and promote carryover of safe, independent performance. Pt performed shower transfer using grab bars with SBA for the majority of the shower, seated on a shower chair. Pt required Min A for doffing all UB garments and demonstrated Mod I for donning and doffing all LB garments. Mod A was required for cleaning the dorsal body surface in adherence to infection control precautions. Pt used 4WW for all functional mobility throughout the session. Graded verbal cueing and hands-on assist were individualized to pt's current functional capacity to advance independence while maintaining safety.
97110/Therapeutic exercise x 15 mins
Pt positioned supine in bed and completed AAROM exercises using a dowel rod: shoulder flexion x 10 reps, followed by bilateral leg lifts x 10 reps, and bridging x 10 reps. Exercises were selected and monitored to address strength and mobility deficits supporting improved independence with bed mobility and car transfers. Monitored pt's movement quality and form throughout to ensure safe execution and appropriate load on targeted muscle groups.
Skilled OT services required this session. Services cannot be safely or effectively performed by the patient, family, or unskilled personnel due to the complexity of the patient's condition and the clinical judgment required to grade assist levels, monitor infection control adherence, and individualize exercise parameters.
Plan and progression
The COTA will advance LB dressing toward increased independence and introduce compensatory strategies for dorsal body surface washing to reduce the required assist level during future sessions.

What’s checked for you
- Minutes checked before you copy; units calculated by the 8-minute rule
- Assist level never guessed — a missing level shows as [Min/Mod/Max] A for you to fill in
- Supervision line on every COTA note
- In-home place-of-service line on in-home visits
- Standard homebound statement when you turn Homebound on
- Maintenance-therapy option
Your discipline and setting, on each note
Choose OT or COTA when you sign up. Every note starts with that discipline — terminology, supervision language for COTA notes, and code selection — and you can switch it on any note.
- Outpatient — in the clinic or in the patient’s home. In-home visits get an in-home place-of-service line.
- Home Health — a homebound toggle you control, decided per visit. Turn it on and the note includes a standard homebound statement.
- SNF — PDPM-aware language framed around Section GG functional areas.
Typed, not recorded
Brevara does not record audio and does not listen to sessions. It is not a scribe. You type what happened after the visit, and Brevara writes the note.
Nothing you document is written to Brevara’s servers; generated notes exist only in your browser session. Brevara is not a HIPAA-covered platform and is not HIPAA-compliant, so do not enter patient-identifying information.
